
If you’ve been for a hygiene appointment in the past and found it uncomfortable, loud, or a bit of an ordeal — you are not alone. Traditional scaling works, but it isn’t kind. Our hygiene team uses EMS Guided Biofilm Therapy (GBT) across the practice, and we wouldn’t go back.

Here’s what it is, how it differs from a standard scale-and-polish, and what the appointment actually feels like.
What is Guided Biofilm Therapy?
Guided Biofilm Therapy is a modern hygiene protocol developed by EMS, the Swiss manufacturer behind the AIRFLOW Prophylaxis Master system. It replaces the familiar metal scraping with a gentle jet of warm water and fine erythritol powder, guided carefully around every tooth, gum line and implant.
The sequence is specific: a disclosing agent stains the biofilm with a colour indicator first, so the hygienist can see exactly what needs removing. Airflow then clears the biofilm and staining without touching the tooth surface aggressively. Only harder tartar — where it exists — is removed afterwards, using a gentler PIEZON instrument.
Is GBT better than a standard scale and polish?
They target the same problem — plaque, biofilm and staining — but they feel very different in the chair.
Traditional scaling uses a metal instrument or a standard ultrasonic scaler, directly on the tooth. It’s effective but it can be noisy, cold and abrasive, particularly at the gum line. GBT uses airflow and warm water, is quieter, and reaches areas (between teeth, around implants, along gum margins) that scaling often misses. For patients with sensitivity, exposed roots, or orthodontic work, the difference is noticeable.
We still use PIEZON scaling where the clinical picture needs it — GBT is not a replacement for every scenario, it’s a better starting point for most.
GBT in practice
A quieter, softer experience than scraping. Most of the work is a warm, fizzy spray of water and fine powder — not metal on enamel. Most patients notice the sensation first, and describe it as closer to a spa than a clean. The disclosing agent shows us exactly where biofilm needs clearing, so we target only what needs removing.
Does GBT hurt?
For most patients, no. Most describe it as a warm, slightly fizzy spray — a few have said it feels like a tooth spa. If your teeth are very sensitive, or you have exposed root surfaces, tell us before we start and we’ll adjust the powder, the pressure and the sequence accordingly. For nervous patients, we build in breaks and keep the chair upright between cycles.
